PrEP Navigator

POSITION SUMMARY:

The PrEP (Pre-Exposure Prophylaxis) Navigator will work closely with the PrEP Manager to ensure proper continuation and expansion of PrEP patient navigation and adherence counseling program at Howard Brown Health (HBH). The PrEP Navigator will work directly with patients seeking consistent access and coverage for PrEP medications. The PrEP Navigator will ensure that participating clients receive relevant HIV/STI-related education and services, sexual health information, and navigation to health insurance if eligible. These services will be geared towards persons at-risk for HIV within the Chicago-land area, including but not limited to cisgender/transgender women who have sex with men, and cisgender/transgender men who have sex with men, and any other persons at-risk for acquiring HIV.

PRINCIPAL DUTIES AND RESPONSIBILITIES:

Assist in the continued implementation of PrEP services and appointment scheduling in primary care for PrEP, while also ensuring that uninsured and under-insured PrEP clients receive education about coverage programs.

Meet with patients weekly time to assist them in filling out PrEP medication coverage applications.

Provide individual- and relationship-based HIV and STI counseling primarily within HBHC’s Electronic Health Records System (EHRS).

Provide education about Post-Exposure Prophylaxis (PEP), and other effective biomedical and prevention interventions; refer eligible, interested clients to primary care medical services at HBH to further discuss PEP and other preventive care with an HBH provider.

Assist in and ensure the training of HBHC HI/STI Prevention and Sexual and Reproductive Health Department and other key patient-facing staff in basics of PrEP.

Coordinate collaborative relationships related to PrEP implementation and service delivery, focusing on agencies located in and around Chicago.

Accurately complete all statistical data forms for PrEP, primarily within the HBHC Electronic Health Records System, ensuring complete and correct reimbursement for services rendered.

Assist Howard Brown Health staff during meetings and trainings which are required by funders, including the Gilead Sciences, the Centers for Disease Control and Prevention (CDC), and the Chicago and Illinois Departments of Public Health.

Review and maintain up-to-date and accurate knowledge of emerging scientific research regarding biomedical HIV/STI prevention strategies, including PrEP, PEP and related interventions.

Maintain highest level of client confidentiality; follow HIPAA regulations.

Attend departmental meetings and trainings.

Perform other duties as assigned.

QUALIFICATIONS, KNOWLEDGE, SKILL REQUIRED:

2-4 years of experience working in HIV/STI prevention or other community health programs preferred.

Outreach engagement and community education experience.

Successful strategies in reaching hard to reach populations.

Must possess strong communication and written skills, as well as a demonstrated ability to work effectively with diverse staff and populations.

Must possess excellent internet and computer skills.
