If you would like to schedule an appointment, or if you have medical questions or need medical documentation, please visit our Scheduling page or call us at 773.388.1600.  

If you have any questions about billing, please call us at 773.896.0940 (Insured Patients) or 872.269.3700 (Uninsured Patients). You can also submit your questions via MyChart by going to “Ask a Question”, selecting “Ask a customer service question”, and then choosing the “Billing or Account Issue” option. For questions about sliding scale fees, please visit our Sliding Scale Procedure webpage.
