CENTERING THE NEEDS OF LGBTQ+ OLDER ADULTS
The goal of the Geriatric Medicine and Aging Services program at Howard Brown Health is to provide inclusive programs and services centering on the needs of LGBTQ+ older adults, community members and Howard Brown patients focusing on overall health and wellness, including physical health, mental health, emotional and social wellbeing, and economic health.
Our services include:
- Chronic disease management, including high blood pressure, high cholesterol, heart conditions, arthritis, and diabetes
- Care consultations
- Memory evaluation and care
- Bone care and osteoporosis evaluation
- HIV/STI testing and treatment
- Infectious disease care, including HIV and hepatitis
Programs
Case Management
The Aging Services Case Management program provides short-term medically focused case management services for patients over the age of 60 living with chronic health conditions and in need of help managing health conditions or using healthcare and insurance systems.
We are able to help you:
- apply for and enroll in public benefits (SNAP, LIHEAP, Medicaid, and Medicare financial assistance programs)
- get durable medical equipment and medication
- access services including food, transportation, emergency financial assistance, and referrals to vision and dental care

This newsletter, published by Howard Brown Health’s Aging Services Program, provides a wealth of information, education, and entertainment that centered LGBTQ+ older adults. Publications include recipes, seasonal tips for healthy living, highlights of Aging Services groups, community member spotlights, LGBTQ+ history, and much more.

Aging Well Archive:
Groups
First Thursday of each month, 6:30 – 8:00 p.m. In-person at HBH Clark.
Rising Phoenix is a Community Group for Long-term Survivors Living with HIV/AIDS. This activity-based group for long-term survivors of HIV/AIDS (diagnosis before 1996) will focus on participant interests as we continue to thrive through our strengths and resiliency.

Meets twice a month. Second Wednesday of each month, 6:30 – 8:00 p.m., and fourth Wednesday of each month, 10:30am – 12:00 p.m. In-person at HBH Clark.
Transcendence in an in-person support group for trans* older adults. This ongoing group is peer-led and will include a variety of activities, as well as opportunities for discussion, mentoring, and resource-sharing in our continued journeys.

Supporting resilience and quality of life for LGBTQ+ adults age 50 and beyond, the Journey Continues is a diverse, gender inclusive and affirming 16 week group process. The purpose of the group is to explore the aging process specific to LGBTQ+ individuals, to build community in a safe and empowering environment and to support resilience and quality of life in the aging process. The group is phenomenological, or client-centered by design, and shaped by the participants of each group.
Tuesdays, 1-2:30pm.
Meets twice a month. First Wednesday of every month from 6:30 – 7:30 pm and third Friday of every month from 12:00 – 1:00 pm. Via Zoom.
Join the Thrive with Pride: Chicago Café in a virtual discussion space where you can share and receive support, learn more about local services and resources, and discuss topics that matter. Thrive with Pride Cafes provide safe and affirming spaces for all older adults, centering on experiences of older LGBT adults.

The Aging Services Advisory Board consists of LGBTQ+ older adult community members and Howard Brown Health staff who work together to improve Aging Services programming and Howard Brown Health services for the older adult community.
