Routine Visit
Establish care with a primary care provider (PCP) or schedule a follow-up visit with your PCP for routine medical care.
Adult Gender-Affirming Care (HRT) Visit
Establish gender-affirming care (including hormone replacement therapy) with a primary care provider.
PrEP Visit
Establish PrEP care with a primary care provider OR schedule a follow-up visit PrEP visit with a primary care provider.
Sick Same-Day Visit
Schedule within the next 24 hours to address acute medical needs. Does NOT include: x-rays and other imaging, casting, etc.
Sexual Health Same-Day Visit
Schedule within the next 24 hours for sexual health needs such as contraceptives and STI, HIV, mpox, and pregnancy testing.
Reproductive Health (RH) Visit
Schedule a new patient or follow-up reproductive health visit for services including PAPs, contraception, and fertility care.
Obstetrics (OB) Visit
Schedule a new patient or follow-up prenatal health visit.
Mpox Immunization Visit
Schedule an adult mpox immunization appointment. All other vaccinations should be scheduled as part of a routine visit.
New Patient Dental Visit (Only X-Rays)
Schedule initial x-rays to become a new dental patient. Does NOT include: routine cleaning, urgent dental care, etc.
Aging Services Visit
Establish care or schedule a follow-up visit with a primary care provider for adults 50 years or older.
