Diabetes Care Management
Diabetes Care Management is a system of support for planning, organizing, and coordinating your care. Working with a diabetes care team can be a rewarding experience that results in significant changes to your health, but it is a commitment. We are looking for highly motivated individuals who want to take an active role in their health. This may include weekly calls from various members of the
team and consistent communication.

Chronic Care Management (CCM)
Chronic Care Management (CCM) is a new type of care that is being put into place by the Diabetes Care Team. The CCM program is designed to help individuals dealing with two or more chronic conditions improve their health by simplifying your care, encouraging collaboration between your various providers, and preemptively monitoring your health on a regular basis.
As a Chronic Care Management (CCM) patient, you are assigned a ‘care manager’ who serves as your point person for all of your chronic health needs: they will work with you to create a proactive, personalized plan to make your overall care more seamless and attuned to your specific needs.
