JOB TITLE: RYAN WHITE PART C MEDICAL CASE MANAGER

FLSA STATUS: FULL-TIME; NON-EXEMPT

POSITION SUMMARY:

As the Midwest’s premier LGBTQ health and human service organization, Howard Brown Health exists to eliminate the disparities in health care experienced by lesbian, gay, bisexual and transgender people through research, education and the provision of services that promote health and wellness. The Ryan White Part C medical case management program at Howard Brown Health plays an essential role in supporting the health and well-being of adult male-identified populations living with HIV, who access medical care at Howard Brown.

The Ryan White Part C medical case manager will be responsible for providing client-centered case management services to adults (≥ 25 years old) and male-identified (cis and TGNC) living with HIV including, but not limited to, assessment of need, service planning, reassessments, treatment education, adherence counseling, linkages and referrals to public and privates services and programs, information dissemination, financial resource management, advocacy, mental health assessment, service plan follow-up, and discharge planning.

 

PRINCIPAL DUTIES AND RESPONSIBILITIES:

Explain the range of Howard Brown Health’s medical and psychosocial services to RWC clients.

Provide information, education, and emotional support to patients/clients in a compassionate, culturally sensitive, and appropriate manner.

Maintain, at minimum, monthly contact with every client on assigned caseload; upwards to 60 clients.

Complete initial assessments and care plans with all RWC clients assigned to the case manager’s individual caseload and conduct subsequent required reassessments every six months.

Participate in ongoing continuing education and maintain current knowledge of HIV-related information by attending trainings, in-services, attending professional development events, and by self-education.

Provide education, information, and referrals to clients; locate, identify, and utilize new social services and resources.

Will follow role assignments that are in alignment with the Patient Centered Medical Home Standards, and participate in all Patient Centered Medical Home efforts.

Evaluate all clients’ ability to understand health plans; provide treatment education, and adherence counseling to clients to improve health outcomes; consult with medical providers regarding assessment and counseling sessions.

Coordinate medical care with providers, schedule patient follow-up (internal and external) appointments; facilitate appointments with collaborative medical partners for provider recommended follow-up screenings and treatment.

Consult and collaborate with a multi-disciplinary team including primary care providers, behavioral health staff, nursing, MA’s, other members of client care teams, partner services staff, and other Howard Brown staff involved in client/patient care.

Assess financial need of clients and facilitate patient financial support; help with applications, especially as it relates to public benefits programs, health insurance (Medicaid/ACA Marketplace), ADAP, and prescription assistance programs.

Provide referrals on an ongoing basis of relevant services, such as: LINK, ADAP, housing, vision, DHS, SSI, unemployment, insurance, rental, and prescription assistance.

Develop and maintain required records, reports, and statistical data. This includes maintenance of an active and inactive caseload, completing day sheets, progress notes, resource receipts, and tracking health maintenance of each client.

Follow appropriate standards of care and support clients with tracking and completing necessary health and wellness screenings including, but not limited to, standard HIV labs, TB testing, vaccines, dental exams, STI testing, and sexual and reproductive health screenings.

Assist with arranging for patient transportation as needed.

Attend and participate in all group consultation/case conferences, social service department meetings and trainings, team meetings, individual supervision, and any other required staff meetings and trainings.

Provide brief case management services on an as needed basis through a rotating “Case Manager on Duty” on-call system.

Assist in the supervision and training of volunteers and interns who provide services to clients as needed.

Maintain the highest level of client confidentiality.

Travel among agency locations based on patient need.

Other duties as assigned.

 

QUALIFICATIONS, KNOWLEDGE, SKILL REQUIRED:

Required:

Minimum of bachelors’ degree in social services field.

Prior case management experience

Experience working in a community-based medical/social services setting

3-5 years of experience in providing case management or prevention services

Experience with Microsoft Suite and data management

Prior knowledge and experience working with clients living with HIV within diverse communities
