Joint Statement from Howard Brown Health and Thresholds
(June 10, 2024 – Chicago, IL) – Howard Brown Health and Thresholds remain committed to our ongoing partnership providing integrated mental and physical healthcare to our 400 shared patients. Many of these patients have very complex behavioral health diagnoses alongside multiple co-morbid primary health conditions. Howard Brown provides both primary care and psychiatry to most of these individuals. Both organizations recognize that the evidence-based model of integrated care is paramount to achieving wellness. To achieve this model, we are committed to ongoing care coordination communication and continued data sharing to jointly monitor health outcomes.
Rooted in a mutual goal of patient-centered care, Howard Brown and Thresholds are ensuring continuity of care for all shared patients. Howard Brown will see patients at their two South Side locations, maintaining the integrated care model even without physical co-location. As a partner with Thresholds, Howard Brown will collaborate on care initiatives to ensure patients can achieve their health goals and will continue to serve as Thresholds’ primary care partner on their newest Certified Community Behavioral Health Clinic (CCBHC).
