(Chicago, IL, August 18, 2016) – Howard Brown Health today published its first Community Impact Report, highlighting patient and community outcomes resulting from its integrated healthcare delivery system.

The progress report uses infographics and text to illustrate how Howard Brown’s individualized approach to healthcare improves the lives of patients, supports the communities of LGBTQ people and allies, and improves overall public health in Chicago. The report describes recently achieved outcomes in reaching patient populations and improving population health as measured by important health indicators.

Highlights include:

  • Howard Brown’s increased reach and scope of its practice, achieved by growing the size and diversity of its patient base and attending to a variety of critical healthcare and wellness needs. More than 19,453 patients accessed care at Howard Brown in 2015.
  • Howard Brown’s HIV prevention and care model, which provides care across the HIV continuum and reached new milestones, including testing services provided to 9,468 people, linkage to care for the vast majority of patients who test HIV-positive (85%), and a majority of patients living with HIV achieving viral suppression (82%).
  • A recap of Howard Brown’s successful bacterial meningitis vaccination campaign, which reached 5,139 individuals, or one-third of Chicago’s total Invasive Meningococcal Disease vaccination campaign in 2015.
  • Howard Brown’s continued innovation of appropriate models of care for transgender and gender non-conforming people, integrated behavioral health service delivery, chronic disease management, robust case management, and, through its Broadway Youth Center, care models tailored for LGBTQ and homeless youth.

“The 2015 Community Impact Report demonstrates how our healthcare delivery system is supporting patients with a range of healthcare needs and health and wellness goals,” said Dr. Magda Houlberg, Chief Clinical Officer for Howard Brown Health. “We present outcomes in three sections, ‘Our Patients,’ ‘Our City,’ and ‘Our Approach,’ showing how Howard Brown works on many levels to improve the quality of life in Chicago.”
