With support from The Baxter International Foundation and Chicago Community Trust, our program for older adults -- AGING AS WE ARE: It's Our Time --is creating a new model of care for LGBT older adults in Chicago. AGING AS WE ARE offers: A Geriatric Medicine Physician Specialist An On-Site Walgreens Pharmacy & Prescription Drug Assistance Mental Health Services HIV/STD Counseling and Testing Caregiver Support for your Partner or Primary Caregivers Legal Assistance including Advance Directives
